We report a rare case of pyoderma gangrenosum misdiagnosed as preauricular fistula infection. To our knowledge, this is the first report to describe pyoderma gangrenosum originating from the site of preauricular fistula. The lesion continued expanding even after combined treatment of systemic antibiotics and thorough debridement.

Common signs of infection include swelling, redness, fluid drainage, and pain. In these cases, treatment typically includes systemic antibiotics. If an abscess is present, it will likely need to be incised and drained. Infected preauricular sinuses/cysts were treated in a consistent manner during the study period. Children presenting with an infected sinus/cyst were treated with oral antibiotics (usually amoxicillin – clavulanate) for 10–14 days. Infected cysts content were needle-aspirated at the beginning of therapy when possible.

Surgical excision is indicated with recurrent fistular infections, preferably after significant healing of the infection. The fistula can be excised as a cosmetic operation even though no In conclusion, a patient with an actively infected preauricular sinus or one with a previous abscess may have a tendency to experience a higher recurrence rate after excisional surgery. Excising a cartilage piece at the base of the tract and demonstrating the sinus with dye injection or with dye injection and probing at the time of surgery will reduce recurrence rates.
